为阐明羌竹清感方及其拆方抗流感病毒性肺炎的作用机制,并为中医“卫气同治”理论截断病毒性肺炎重症发生提供科学依据。本研究采用A/PR/8/34病毒气管滴注构建致死性病毒性肺炎小鼠模型。以一线抗病毒药物磷酸奥司他韦为阳性对照,评价了羌竹清感方及其拆方(九味羌活汤、竹叶石膏汤、九味羌活+竹叶石膏汤和余方)对模型小鼠生存天数、肺组织病理、肺组织病毒载量和TNF-α、IFN-γ、IL-6炎性因子含量以及肺泡灌洗液中巨噬细胞、中性粒细胞和NK细胞比例的影响,同时探索羌竹清感方及其拆方对细胞死亡的抑制作用。结果显示,羌竹清感方可以显著延长模型小鼠生存期、减轻肺组织病理损伤、降低肺组织病毒载量和炎性细胞因子的表达、减少固有免疫细胞募集以及抑制巨噬细胞死亡。这些结果证实羌竹清感方通过多途径发挥治疗作用,其整体疗效优于各拆方组,从而为“卫气同治”理论在截断病毒性肺炎重症化方面的科学价值提供了有力证据。
Abstract:This study aimed to elucidate the mechanism of Qiangzhu Qinggan Formula(QZQG) and its disassembled formulas in treating influenza viral pneumonia, thereby providing scientific evidence for the "Defense-Exterior and Qi-Aspect Simultaneous Treatment" theory in preventing progression to severe disease. A lethal viral pneumonia mouse model was established by intratracheal instillation of the A/PR/8/34(H1N1) strain. Oseltamivir phosphate, a first-line antiviral, was used as the positive control. The effects of QZQG and its disassembled formulas—Jiuwei Qianghuo Decoction(JWQH), Zhuye Shigao Decoction(ZYSG), JWQH combined with ZYSG, and the remaining components(YF))—were evaluated in terms of survival time, lung histopathology, pulmonary viral load, levels of inflammatory cytokines(TNF-α, IFN-γ and IL-6), and the proportions of macrophages, neutrophils, NK cells in bronchoalveolar lavage fluid(BALF). In addition, the inhibitory effects on virus-induced macrophage death were assessed. The results demonstrated that QZQG significantly prolonged survival, attenuated lung pathology, reduced viral load and cytokine expression, decreased innate immune cell recruitment, and suppressed macrophage death. These findings indicate that QZQG exerts therapeutic effects through multiple pathways, with overall efficacy superior to all disassembled formulas, thereby providing strong evidence for the scientific value of the "Defense-Exterior and Qi-Aspect Simultaneous Treatment" theory in preventing the progression of viral pneumonia to severe stages.
