为探究Epstein-Barr病毒(Epstein-Barr,EBV)感染患儿异型淋巴细胞比值和外周血淋巴细胞(Peripheral blood lymphocyte,PBL)比值与EBV-DNA载量的关系,选取2022年10月~2023年12月广州市红十字会医院收治的186例EBV感染患儿作为此次研究对象,运用RT-qPCR技术检测其外周血EBV-DNA载量,并根据EBV-DNA载量将患儿分为EBV-DNA阳性组和EBV-DNA阴性组,检测患者血常规,统计分析淋巴细胞数据,并通过显微镜镜检外周血涂片,计数异型淋巴细胞,最终计算其在淋巴细胞中的百分比。应用Pearson相关性分析EBV感染患儿异型淋巴细胞比值和PBL比值与EBV-DNA载量的相关性。结果发现,共有73例患儿EBV-DNA呈阳性,阳性率为39.25%,且EBV-DNA阳性组载量显著高于阴性组(P<0.05);不同性别间EBV-DNA阳性率无显著差异(P>0.05),但1~3岁患儿阳性率显著高于其他年龄段(P<0.05);进一步分析发现,EBV-DNA阳性组患儿的异型淋巴细胞比值和PBL比值均显著高于阴性组(P<0.05);Pearson相关性分析揭示,随着EBV-DNA载量增加,异型淋巴细胞比值和PBL比值均呈上升趋势,且两者与EBV-DNA载量均呈正相关。本研究表明,EBV-DNA载量与异型淋巴细胞及PBL比值密切相关,且1~3岁患儿EBV-DNA阳性率显著较高。因此,监测上述指标对于EBV感染患儿的临床诊断、病情监控及制定年龄特异性治疗策略具有重要意义。
Abstract:This study aims to investigate the relationship between the ratio of atypical lymphocytes and peripheral blood lymphocytes(PBL) and EBV-DNA load in children infected with Epstein-Barr virus(EBV).A total of 186 EBV-infected children admitted to Guangzhou Red Cross Hospital from October 2022 to December 2023 were selected as study subjects. The EBV-DNA load in peripheral blood was measured using RT-qPCR, and the patients were divided into EBV-DNA positive and negative groups based on the EBV-DNA load. Routine blood tests were conducted, and lymphocyte data was statistically analyzed. Peripheral blood smears were examined through microscopy to count atypical lymphocytes, and their percentage in the lymphocyte population was calculated. Pearson correlation analysis was performed to assess the relationship between the ratios of atypical lymphocytes, PBL, and EBV-DNA load in EBV-infected children. The results revealed that 73 cases were EBV-DNA positive, with a positive rate of 39.25%. The EBV-DNA load in the positive group was significantly higher than that in the negative group(P < 0.05). No significant difference in EBV-DNA positivity was observed between sexes(P > 0.05); however, the positive rate in children aged 1 to 3 years was significantly higher than that in other age groups(P < 0.05). Further analysis showed that the ratios of atypical lymphocyte and PBL in the EBV-DNA positive group were significantly higher than those in the negative group(P < 0.05). Pearson correlation analysis indicated a positive correlation between EBV-DNA load and the ratios of atypical lymphocytes and PBL, with both increasing as the EBV-DNA load increased.This study demonstrates that EBV-DNA load is closely associated with the ratios of atypical lymphocyte and PBL, and that children aged 1 to 3 years exhibit a significantly higher EBV-DNA positivity rate. Monitoring these indicators is therefore crucial for the clinical diagnosis, disease monitoring, and development of age-specific treatment strategies for EBV-infected children.
