肠道病毒(Enterovirus,EV)可以引起包括手足口病的全身各系统的病症,柯萨奇病毒A组2型(Coxsackievirus A2,CV-A2)在世界范围广泛流行。我们对2018年北京市丰台区一起聚集性发热疫情进行病原学鉴定和基因特征分析,为该类疫情的防控提供实验室支持。采集6例病例的咽拭子标本,进行EV实时荧光RTPCR检测,结果显示5份为EV阳性。使用人横纹肌肉瘤细胞进行病毒分离,分离到4株EV,通过EV分支分型方法,确定这4株EV为CV-A2。4株CV-A2分离株的VP1区核苷酸序列和VP4区核苷酸序列相似性高达99.5%~100%,基因进化树图分析显示,4株CV-A2与D基因型代表株处于同一分支,与2017年中国天津分离株MG926808核苷酸同源性最高,为99.1%~99.3%。根据以上结果,确定此次聚集性发热疫情是由CV-A2感染引起,且属于在我国流行的D基因型,提示我们在以后的工作应结合流行病学资料,加强对CV-A2的监测。
Abstract:Enteroviruses(EVs)are important etiological agents that can cause a wide spectrum of diseases,include hand,foot,and mouth disease(HFMD). The prevalence of Coxsackievirus A2(CV-A2)has been reported worldwide. We identified and analyzed the pathogen genetic characteristics of the cluster fever occurred in Fengtai District of Beijing,China in 2018,and provided valuable information for prevention and control of this disease. Six oropharyngeal swabs were taken from six patients. Real-time reverse transcription-polymerase chain reaction(RT-PCR)was performed and five samples were enteroviruses positive. Four strains of the virus were isolated from original clinical specimens by propagation in human rhabdomyosarcoma(RD) cells. four strains of coxsackievirus A2 were identified by branch typing. The homogeneity of VP1 and VP4 nucleotide sequence of the four strains of coxsackievirus A2 were from 99.5% to 100%. Phylogenetic evolutionary tree result indicated that the genotype D was responsible for the four strains of coxsackievirus A2. According to these experimental results,the epidemic of cluster fever in Fengtai District of Beijing,China was caused by coxsackievirus A2,which belonged to D sub-genotype and persisted circulating in mainland of China. The situation suggests that we should enhance coxsackievirus A2 surveillance and combine epidemiological data.
