严重急性呼吸道感染(Severe acute respiratory infection,SARI)是引起婴幼儿就诊和住院的常见原因,也是该年龄段人群发病的主要原因,而病毒感染占呼吸道感染病例的60%以上,其中人副流感病毒(Human parainfluenza viruses,HPIVs)是引起呼吸道感染的常见病原。为分析河南省漯河市儿童严重急性呼吸道感染(SARI)病例中人副流感病毒3型(HPIV-3)检出状况及血凝素-神经氨酸酶蛋白(Hemagglutinin-neuraminidase,HN)基因特征,采用实时荧光定量聚合酶链反应(Polymerase chain reaction,PCR)方法对河南省漯河市中心医院于2017年10月10日至2018年12月27日送检的627份SARI病例咽拭子进行核酸检测,筛查HPIV-3阳性标本,对HPIV-3阳性标本用反转录聚合酶链反应(Reverse transcription polymerase chain reaction,RT-PCR)扩增其HN基因,并对扩增产物进行序列测定,与GenBank数据库中的国内外HPIV-3 HN基因进行对比分析,绘制系统进化树,并分析基因特征。627份咽拭子标本中,HPIV-3阳性有27份,检出率为4.3%。59%的HPIV-3阳性病例分布于春夏季,发病人群中,6岁以下儿童占96.3%。通过RT-PCR方法共获得18株HPIV-3的HN基因,经过序列测定及对比分析可划分为两个基因亚型:C3a和C3b。C3a亚型有10株,C3b亚型有8株。18株HPIV-3的HN基因序列的核苷酸和氨基酸同源性分别为97.1%~100.0%和98.8%~100%。与原型株Wash/47885/57相比,本次扩增18株HPIV-3 HN基因序列的核苷酸和氨基酸同源性分别为94.5%~95.0%和97.0%~97.7%。与GenBank中国内外的HPIV-3 HN基因的代表株比较,其核苷酸及氨基酸同源性分别为94.1%~98.5%和96.8%~97.0%。本研究提示HPIV-3是河南省漯河市儿童SARI病例中检出的病原之一,2017年10月至2018年12月期间,至少有C3a、C3b两个基因亚型的HPIV-3毒株在漯河市共流行。
Abstract:Severe acute respiratory infection(SARI)is a common cause of hospitalization for infants and young children. It is also the main cause of the disease in this age group,and viral infection accounts for more than 60% of respiratory infections. Among them,human parainfluenza viruses(HPIVs) are common pathogens causing respiratory infection. To investigate the infection and hemagglutinin-neuraminidase(HN) gene characteristics of HPIV type 3(HPIV-3)in SARI cases among children in Luohe,Henan Province,China,real-time PCR was performed to screen HPIV-3 positive specimens collected from patients with SARI in Luohe Central Hospital from October 2017 to December 2018. HN gene was amplified by reverse transcription-polymerase chain reaction(RT-PCR). The HN gene was sequenced and compared with representative strains in the GenBank to generate the phylogenetic tree of nucleotide sequences. The results showed that 27 of 627(4.3%)specimens were positive for HPIV-3 by real-time PCR,59% of HPIV-3 cases were distributed in spring and summer,and children under 6 years old accounted for 96.3%. A total of 18 HPIV-3 HN gene sequences were obtained by RT-PCR and belonged to the genotypes C3 a and C3 b,with 10 strains of subtype C3 a and 8 strains of subtype C3 b. The nucleotide and amino acid identity were 97.1%~100.0% and 98.8%~100%,respectively. Compared with the prototype strain Wash/47885/57,the nucleotide and amino acid homology of the amplified 18 strains of the HPIV-3 HN gene were 94.5%~95.0% and 97%~97.7%,respectively. Compared with the HPIV-3 HN gene of the representative strains in GenBank,the nucleotide and amino acid homology were 94.1%~98.5% and 96.8%~97.0%,respectively. HPIV-3 is one of the pathogens detected in children with SARI in Luohe,Henan Province,China. During October 2017 to December 2018,at least two HPIV-3 strains with subtypes C3 a and C3 b were prevalent in Luohe,China.
